Dear Student Disability Services,

Please confirm receipt and open an SDS case/ticket for accommodations
for my requested name-clearing hearing.

I’m invoking the ADA/§504 interactive process now so accommodations
are in place when the hearing is scheduled.

The attached **Exhibit A (Excerpt — Diagnoses and Functional
Accommodations)** summarizes my neuropsychological evaluation.

It documents ASD, ADHD, and GAD with associated functional limitations
in attention, processing speed, working memory, and social
communication, as well as specific educational/testing accommodations
relevant to a hearing format.

Based on those findings, I request the following hearing accommodations:
• Equal total time with the University
• A 10-minute break every 30 minutes (or as needed)
• Seven-day mutual exchange of exhibits and witness lists
• Fair questioning method (live or written via neutral presider) with
managed pacing
• Permission to consult written notes and pause briefly to review
• Real-time captioning (CART) if requested
• Neutral presider and option for remote appearance if clinically indicated

My clinician’s supporting letter is being finalized and will be
provided promptly.

Please proceed with the interactive process, confirm the SDS case
number and assigned coordinator, and advise if any additional
documentation is needed.

EXHIBIT A — NEUROPSYCHOLOGICAL EVALUATION (EXCERPT)
(Only diagnoses, functional limitations relevant to hearing, and accommodations)

DSM-5 / ICD-10-CM DIAGNOSTIC IMPRESSIONS:
F84.0 Autism Spectrum Disorder, Mild (formally considered Aspergers)
F90.2 Attention-Deficit/Hyperactivity Disorder, Combined type, Severe
F41.1 Generalized Anxiety Disorder, Severe

Functional limitations (quoted/condensed from evaluation):
Results of cognitive functioning revealed that Dr. Bass displays very superior skills in crystalized knowledge (99.7th %ile), nonverbal reasoning (99th %ile), and academic achievement (84th+ %ile). Areas of general weakness are attention and executive functioning (63rd %ile), immediate, working, and delayed memory (16th–63rd %ile), and social communication and interpersonal dynamics. Deficits in processing speed, memory, and executive control coincide with the pre‑existing diagnosis of ADHD.

Selected accommodations from the evaluation (education/testing) relevant to hearing format:
• Testing in a distraction‑free environment.
• Extra time (1.5×) and extra breaks; standing/stretching as needed.
• Permission to record lectures.
• Extended time for in‑class writing assignments.
• Copies of lecture notes and visual aids.
• Use of handheld calculator for assignments and tests.
• Use of computer for taking notes.
